EBS PW acting up

Have any of you experienced the issue of PW acting up? We are on EBS 12.0.4 with CRM module. The system is fairly stable, but from time to time, we have a user reporting that he has to have his pw changed on daily basis, sometimes even several times a day. We did issue a SR couple months ago and was told to disable the 'pw assistant' button which we did. We thought we would be home free from that point on, but NO....... It just happened again last week. Any advice or suggestions will be greatly appreciated.
