datafiles defragmentation

We have datafiles that our disk Defragmentor software  shows that they have between 100,000  To 600,000 Fragments

Questions : 1. What might cause this fragments
                    2. How to fix ?

We know it's not tablespace fragmentation.

I am asking for your advise , regarding the CAUSE of so many fragments.

 Could it be due to wrong INITIAL  or NEXT extend size ?  What are other  possible causes? How do you recommend to fix?
