Something is triggering duplicate emails to be generated in a worklow

Hi worker bees,

I have created a workflow for the communications wrapup layout for CCOD. the workflow is [<bEmail_Acct_Owner_ITAG>]='Y'.  It is a checkbox field called Email Acct Owner. When it's checked, the workflow will trigger an email. Funny, it was working the other day, and now it's not working. Anyway, when it was working it was creating duplicate emails. There is no conflict with other Activity workflows, as this is the only one triggering an action.

Any idea as to how I get two emails from being generated?
