Reporting on Survey Results

I am working with a firm that sells cold-calling/inside sales services and calls on behalf of large companies in the telecom products & services space--that is companies mainly selling to the telecom companies. A couple of things I wanted to learn from someone:

1. I am familiar with the assessment scripts in CRM On Demand but has anyone ever been able to report results from those assessments/surveys?

2. The firm I mentioned above runs sales campaigns on behalf of their clients and often the qualification survey (questionnaire) is unique to each of the sales campaigns the firm runs. What would be the best way to capture/record and report on these unique set of questions? I can't imagine creating new fields in say the Leads object for every new survey is a very repeatable process...any thoughts?
