What is the best syntax/process to create the index on this table?

I am attempting to create an index on a highly written to table consisting 50million+ rows.  I cannot stop the processing of inserts on this table.  What is the best syntax/process to create the index on this table?

Database 11.1.0.7
OS          Microsoft Windows x64 (64-bit)
