Faulting application ons.exe, version 0.0.0.0, faulting module ons.exe.

 I set up a two node Oracle 11.1.0.6 x64 rac on two Windows 2003 servers. The application event log shows the error “Faulting application ons.exe, version 0.0.0.0, faulting module ons.exe, version 0.0.0.0, fault address 0x0000000000006039.” And  a crs_stat –t displays the following with ons offline on node oraractstb.
