Incomplete records written to the F08042 Employee History file?

Does anyone know why some records write to the history incompletely? We have some records written that don't have the User ID nor the Program ID written on them. For SOX purposes, we need to review all changes made to the employee master and by whom.  We're on Xe. Most records that seem to be missing these fields were for pay cycle code (PCCD); a couple were for SSN. Has anyone seen this happen before?
