Proxy database authentication management

It is my understanding that I can create a single database user (a proxy) for my middle tier to use for database connections.  And if the middle tier connects as the proxy account but specifies the application user, the application user's identity will be preserved at the database level. This all sounds great, but from the examples of this that I've seen so far, I still have create one database account for EVERY application user, correct? If that is true, what is the advantage of proxy users?
