Performance Management: Is there a seeded performance appraisal report ?

 For R12.1 HCM Performance Management - the business has a need to print out a hardcopy performance appraisal report (customizing with logo image and other custom text sections) that can be signed after the manager has entered the Performance appraisal in manager self service, submitted it for approval, and the appraisal has been approved. 

I do not find a seeded performance appraisal report.  Does anyone have any recommendations on where an appraisal report can be found or what would be the best way to make this accessible for print for a manager in self service repsonsibility?

There is a printable version of the appraisal available as the manager is entering the appraisal; however it is not in the format nor does it contain all the information that is needed for the hardcopy appraisal report.
