9.3.1 Error when adding attachments

OAS 10.1.3.1 patch set 4

DB 11.2.0

I have installed the above stack on Linux RH 5.4 following the documentation. All is well except adding attachments in the Web client documents up load OK but are not displayed in the attachment table. If I use the java client upload works but is followed by this error message PCException [errorType: 0 errorCode: -1 errorMessage: ORA-29861: domain index is marked LOADING/FAILED/UNUSABLE ]

I have verified the files are in the agilevault, confirming the upload has worked, but in either client they are not shown in the attachment view / table for the object.

Very new to Agile, so a simplified step by step fix appriciated.
