Request ID Not Showing in Request Centre

We recently had a call from a user advising that they ran a report through the request centre in ADI but could not see it running and now cannot see ir in the completed tab of the request centre either.

To get around this the user simply runs the report again and you can see the request and publish the report in the desired format. I was just wondering if anyone else has experienced this and if so how did you fix it?
