Opatch failed with OUI-67124:Re-link fails on target "ioracle".

Hi,

 I am getting the below error message when apply the October cpu PATCH(Oct CPU'10) on 10.2.0.4 database.

 

  "Opatch failed with OUI-67124:Re-link fails on target "ioracle". "

After applied the patch, I started the db in upgrade mode and run catbundle.sql cpu apply on the database.It had failed with the error message "  "Opatch failed with OUI-67124:Re-link fails on target "ioracle". "

Please suggest me on this.
