Upgrading From EBS 11i to R12 - Upgrade Project Plan

Hello Experts,

We are in process of Upgrading from 11i to R12.

Can any one share their Project PLan for Upgrade activities, which covers :

PLanning, Review & Design, Build & test (Functional & technical, Including upgrading of Custoimizations), Iterations,Internal UAT,   User Acceptance Testing, Transition Planning , Cutover, Post upgrade support.
