Setup of ASM/Grid in 11g

 We are setting up a new stand alone server running Windows 2003 R2 std x64. Its  a Proliant DL380 G7 with 6 disks. 2 will be used for the OS (hw Raid mirror) and and apps with the remain 4 for db files.   I'm a little confused in settup up the ASM/Grid infracture in this regards.   Do I first setup the disks at the bios or OS level then stamp them for grid?   

In trying to follow Oracles recommeded OFA, it is not clear how this is setup.

We will also be upgrading an exist Windows 10gr2 install to 11g.  This setup did not follw Oracle OFA.   It has 1 disk mirrored for the OS and apps (where Oracle is installed) and 3 drives (disks hw mirriored) for the data disks.  How do I get from this to 11g using OFA/grid or shoul;d I stay using NTFS file system? This is a production box so downtime needs to be minimized.
