R12 - LAB Organization transfer from OPM to Discrete

During the Pre-Upgrade process we did not transfer the LAB Organization from OPM to Discrete. All other Pre-Upgrade steps went through fine without errors. However we landed up with more than 800000 errors logged just for LAB not being transferred and it's impact to the Quality Samples.
I was trying to go to the Convergence Migration Setup and setup a new organization for LAB with
Type "Labroartory"
Migrate as "New"
Organization Name as "LAB:Default Lab"
Inventory Org "Selected"
Default Status as "DFLT"
Org as "LAB"
Process Enabled as "Enabled"
However Template Org does not have any values to select from
