409a Setup

 Could someone supply the 409a setup on the US Leg screen on the PDBA setup and which special handling codes it should be attached to in the W2 Workbench?

When we flag it with a Y it displays on the W2.  When we add it to the Special Handling for Deferred Comp it does not display on the W3.
