Configuring webutil as part of E-Business suite

We are trying to configure Webutil as a custom app to be run as part of our E-business suite. For starters, we have registered WEBUTIL_DEMO.fmx as a custom app to test it out. We have been encountering bean errors when I press the 'Get Client Info' button. It throws errors 'oracle.forms.webutil.clientinfo bean not found ...','oracle.forms.webutil.CAPI.CAPIFunctions bean not found ...' etc

The form server is 10.1.2.3.0. We have applied bundled patch 9593176 on top of it.  

We have completed all the documented configuration steps. Running Webutil Diagnostic test form as per 553849.1 shows the error below
