Quantity requested shows twice requested amount on pick slip

Has anyone ever seen the quantity requested for an item, show up doubled on a pick slip?

I checked the IR, action history, supply/demand, order, pick release, pick confirm, material transactions, conversions in units of measure/issue, item setup, etc.

The IR requested a quantity of 10 boxes and I see that throughout the process - but suddenly the pick slip requests 20 boxes.  Nowhere else references 20 boxes.

Any thoughts?
