Dashboard Prompt using Activities custom field failure

 I need to generate a dashboard prompt on Active Subject Area: Activities, and initially tried using an indexed check box and got

Error Generating Drop Down Values Odbc driver returned an error (SQLExecDirectW).  Error Codes:  OPR4ONWY:U9IM8TAC:012DL65P

I finally gave up on the check box and changed to a pick list I am again getting this same error.

This error is occurring when viewing the dashboard prompt before it is saved so it isn't even getting to a problem with the fx. 

I need to pull a weekly activities report which has items that the user feels their manager should know about, and that user feels are important enough to be marked for executive review.   The manager will review what the user has included and add or remove the executive review indicator as appropriate.
