Despite approving the PO approval going to next level approval

Hi,

A PO sends notification to its approver .

The approver clicks on approve button ,but still the  notification goes to next level approver.

4 next level approvers have approved it ,but it keeps on going up the herirarchy.

the amount is not much just 100$.

When i query PO_ACTION_HISTORY table ,it shows all the rows with action FORWARD .

What might be happening?
