Upgrade Project Plan

Hello, All.  I am in the process of upgrading from 11.5.10.2 to 12.1.3.  Does anyonehave a upgrade project plan they wouldn't mind sharing.  I'm just looking for something that will give me an idea of the different task I should include in my plan.
