BEA-000627 Reached maximum capacity of pool

Environment: Oracle 11g Fusion Middleware environment with 10.3.2 Weblogic (and 11g database). We are experiencing "Reached Maximum capacity of pool" lately even though no of sessions in database doesn't match exactly. The current settings for the data source are as follows:

Initial Capacity: 15
Maximum Capacity: 55
Test Frequency: 120
Test Table Name: SQL SELECT 1 FROM DUAL
Seconds to Trust an Idle Connection: 10
Shrink Frequency: 900
Maximum Waiting for Connection: 2147483647
Connection Reserve Timeout: 10
Statement Timeout: -1
Ignore In-Use Connections: True
Remove Infected Connections Enabled: True

I had to increase the Max capacity to higher number couple of times to overcome the issue. I also had to do a "Force Shutdown" of the data source to release any unknown connections out there. I observed that logs displayed messages suggesting that it released around 98 sessions (compared to max capacity of 55).
