USB Hub is not taken offline after it is disconnected leading to device failure when reconnected.

I have a USB 2.0 KVM plugged into the USB 1.1 root hub ports on a V240 and until a recent patch update all was working well.  However now the USB device is recognized on boot but when the KVM is switched the HUB is not offlined.  This means no reset is sent when the KVM is switched back.  If I remote in and do cfgadm -x usb_reset usb0/1 then I see the offline messages followed by the online messages.

I believe it has something to do with the hubd driver but I don't know how to manipulate this driver such that it will offline the hub when the hub is disconnected.
