Adding a node to RAC 11.2.0.2, root.sh fails

We are adding a fourth node to our Oracle RAC, CRS and ASM are 11.2.0.2.  OS is RHEL 4.8.  Root.sh on new node is failing and it appears that it cannot start the CRSD.  CRSD is online with a state of INTERMEDIATE.  Has anyone experienced this before?  How do we fix this?  We tried to stop/start crs -f and rebooting.
