Recurring Node & Instance Evictions on New 4-Node 11.2.0.2 RAC Cluster

We have recently built a new 4-node RAC cluster on RedHat 5.5 w/ 11.2.0.2 RDBMS and Grid Infrastructure.  Our interconnect is configured as a failover bonded pair of GigE ports one two different NICs.  We use ASM for the voting disks, OCR, and database files.  We are not using ACFS at all.  We have two databases running, each with four instances, one on each node.

Since completing the build out of this cluster we have had ongoing stability issues where nodes will be evicted and rebooted, and also see recurring ASM instance evictions which in turn cause RDBMS instances to crash. 

We have multiple Sev1 SRs open with Support on these problems but have not made any progress for several months.
