ORA-01274 From the Standby; The Recovery Continued

Hello DBAs:

I have a 10.2.0.4 ASM managed single instance physical standby database.  The primary is also ASM.  Both are on AIX 5.  The Data Guard broker is not started, and we ship archived logs, so not Real Time Apply.

The standby parameter standby_file_management was set to "manual", so when a new datafile was added to the primary, the standby reported ORA-01274.  This should have been fixed right away, but the media recovery continued without interruption, so we did not detect the problem.  It's clear we are not checking the standby thoroughly enough.  There were no gaps reported by v$archive_gap, and the logs applied to the primary were also applied to the standby, except for the missing file.  When we tried to activate the standby (for a DR drill), it failed.
