Duplicate error writing to F986114A

This week, a particular UBE decided to start showing an error in the logs while running.  It completes ok. It seems to write to the F986114A, but it is telling us it is not.  One thing I have noticed is it is not writing the Interconnect or 4 line to the file.  I know what the file's purpose is.  I am trying to understand how it just started having this 'error'.  There has been no updates to objects or tools or anything like that recently.  Has anyone seen this error and what have you done to fix it?
