Questionnaire for Oracle Financials Upgrade or Reimplementation

Hi,

We are looking a questionnaire based on which we can decide whether we should upgrade or reimplement from 11.5.10 to R12. We currently have GL, AP, AR , FA and Service Contracts. Please let us know if you have any any such questionnaire and share the same.
