How do I setup Advanced Inbound to Screen pop the Contact Center form using values other ANI when in

In the CallCenter>>IVR page (Call Center HTML Administration resp), we have mapped Customer Number to a field in Cisco (IVR)....specifically, the Caller Entered Digits field, but the form still pops using the ANI.  The question here is, if we are in passive mode, is there another setup that needs to be done outside the IVR setup (or is IVR setup not necessary)?  Is there a default profile option (IEU, CS, CSC, Service, CCT) that should be set?  During IVR setup, it is suggested that the middleware not be selected from LOV, and only use the Server Group info.....can anyone confirm
