WF Error in activity Launch Create PO Workflow

Dear all!
Has anybody faced with error in activity Launch Create PO Workflow for item type "PO Requisition approval" and does anybody know how to eliminate it
Error Message 3146: Commit happened in activity/function
'MAIN_REQAPPRV_PROCESS:LAUNCH_CREATE_PO_WF/PO_REQAPPROVAL_LAUNCH.LAUNCH_CREATEPO_WF'
Error Stack Wf_Engine_Util.Function_Call(PO_REQAPPROVAL_LAUNCH.LAUNCH_CREATEPO_WF, REQAPPRV, 703025-887785, 202082, RUN)

The steps we get this error are:
1. The requisition is created in iProcurement, CPA is selected
2. The PO Requisition approval fails to launch the CREATEPO workflow and the exception appears
