User Specific - Missing Project Activity Codes and Project Specific Layouts

A user in our P6 (6.2.1) database is unable to view project specific layouts.  We have experienced this issue in the past, and other users in the same schedule experienced the same problem too.  The “fix” to log out and log back in, usually enables the user to view the project specific layouts again.  In this case, after several attempts to log out and log back in, opening the same project schedule, there are still no project specific layouts. 

This user is also experiencing the inability to view project activity codes for the same project schedule.  Primavera provides a pop-up box that states “No project activity codes have been defined.
