Security Failing on Self Service Pages

Has anyone else run into any issues with security failing on the self service pages on Oracle 12.1.3?  Our end users use self service (framework) pages to enter and view data, and occasionally security seems to fail which allows them to have access to a population that they don't normally have access to via their security profile.  We cannot reproduce the issue and therefore we are not able to pinpoint the cause of it.
