Can't see the disk - SF V440

Hi, Having a problem with SF V440 have 3 disk on target 0,1 & 2, I can't see the disk 0 with format command. I did all the possibilities like devfsadm -Cv then devfsadm then cfgadm -al tried to reconfigure with cfgadm also I did the reset-all probe-scsi-all at OK prompt with auto-boot false even though I used the new disk as well.

Also I upgrade the FW but nothing happened still i can't see one of the disk with should be c1t0d0s0

c1::dsk/c1t2d0 disk connected configured unknown
