Report showing XML Output in place of PDF sometimes - I see XML tags

Report showing XML Output in place of PDF sometimes, I see XML tagsThe interesting is that the issue is not always, is sometimes.
On the Requests form itself, where we see the request ID's , View Details --> Options , so I see the Layout being specified, I was suspect the Layout is not specified, but is there.

What could be else? Why the issue is not always ?
I uploaded the "Example_correct_incorrect_output.doc" to have a clear picture about this issue.

Any hints please?
