Error when trying to create payment in R12

Hi,
Step :
1-payables manager responsablitity
2- payments > create payments
3- the first payments pass without error.
4- create second payments, message error show : execption occured when performing payment creation.
wordaround :
1- close payment form, and try agin.

please help me.
