LCM Account FORM not displayed >> Receiving Options of Organization Setup

Hi,

Application version and release is R12.1.3.

While trying to make setup for LCM, the actual Form don't have the fields needed for entry of LCM relevant accounts, as shown in the attached form (documentation). Both Forms (Documentation and the Actual available in the system) are uploaded for review and advice. How can that be made available?
