How to recover boot-archive Solaris 10

Dears,
Can any one please let me know that how to recover boot archive? I tried several time with "bootadm update-archive -R /a" but didn't get any luck always system coming with following errors -

WARNING: Automatic update of the boot archive failed.

Update the archives using 'bootadm update-archive'

command and then reboot the system from the same device that

was previously booted.

 

Sep 15 16:06:06 svc.startd[7]: svc:/system/filesystem/usr:default: Method "/lib/svc/method/fs-usr" failed with exit status 95.

Sep 15 16:06:06 svc.startd[7]: system/filesystem/usr:default failed fatally: transitioned to maintenance (see 'svcs -xv' for details)
