Version upgradation issue - 12.0.4 to 12.1.3

Version upgradation issue:

Issue description: ORA-01403 no data found --FRM-40735: POST-UPDATE trigger raised unhandled exception ORA-06508. Error raised during the Item master updation, creation and misc issue , receipts etc after migration 12.0.4 to 12.1.3

Error raised while saving, updating the Items in Item master screen.

Any help on this issue is really appreciated.
