Encountered unexpected system error

Hello,

Am facing error at attachment page under Requisition & Purchase Modules, when i want to attach, the file is loaded successful but when i click submit i got this error
"

Error Page

You have encountered an unexpected error. Please contact the System Administrator for assistance.
"
