How to eliminate False Default password "Clear" policy alert.

We are getting lot of alerts for default password "clear" alerts and after some time again violation alert. I found that we are actually violating the default password policy for some of the accounts. but clear and warning alerts keeping coming.

I am not sure as how  Clear alert is coming when still policy vbiolation is not cleared by taking appropriate action.

Appreciate quick hint/help. Thanks
