Where can I find concise documentation on How to do an out-of-place upgrade with RAC/grid architectu

We have several production RAC databases that need to be updated from 11.2.0.1.0 to 11.2.0.3.0.  And now since the updates are full installs, oracle recommends doing the out-of-place type of upgrade.  Where can I find some thorough documentation on doing this type of upgrade, particularly that deals with upgrading the grid as well ??
