US PAY: Element setup to feed Box 12 - Healthcare costs

Hi Everyone,

My client uses Oracle Advanced Benefits and Oracle Payroll.  We are setting up the Standard Rates in OAB to pass the per pay period amount of the health care coverage costs for the medical plans.  However, I'm not sure what Primary Classification to use on the Element, what balance to feed, etc.  Is anyone willing to share their setup on the Payroll side?  Thanks.
