Automatic Task generation does not work from Service Request Tab in eBusiness Center (R12)

I have defined a task template with 2 tasks.  Each task type is mapped to Service Request & Task Manager in the CS App.  The task template is mapped to the S/R request type.  The S/R Type is mapped to the custom responsibility.

Yet when I create the S/R using the S/R Type in the eBusiness Center --> Service Request Tab, the 2 tasks are not generated. 

When I create the S/R using the S/R Type in the Contact Center legacy form, the tasks are automatically generated

When I create the S/R using the S/R Type in the Customer Support Agent Dashboard, the tasks are automatically generated.
