Naive question on /etc/hosts in face of FQDN DNS names

Hello,
By the way, what should be the reasonable (or more generally usual/correct) configuration of /etc/hosts?

Should a FQDN be listed as an alias or should it be the first name right after the IP on each line of /etc/hosts?
