R12.1.3 Lease Center Vendor/Investor Disbursement Forms Functionality

Lease Center Vendor/Investor Disbursement forms are not working in our internal instance EADVI2.  I found bug "9827271:  TST1213:OKLCSVSC: Vendor/Investor Disbursement Forms Not Working Completely" and this appears to be the same issue.

 

Are there additional setups required to enable this functionality.  Please share any documentation you have on this functionality in R12.
