ACH File

Has anyone experienced their ACH file to sometimes split?  Over the last month we have had it happen twice where the header record falls in the body of the bank file - everything appears to be o.k.  I check the employee it split on and the setup is fine.  All the data is there so I cut and paste to put the file back in order but I need to find out why it's happenning?
