Want 'pritner' and 'no. of copies' as concurrent's user defined parameter

Hi

Instead of using the options button before submitting the request, users want to select the pritner and no. of copies as concurrent's parameter.

How is this possible?

Also, we require that 'Submit another request (Yes / No)' window to be supressed so that the parameter values submitted never get defaulted in subsequent submission of same request
