Upgrading from Release 12.0.4 to Releast 12.1.3

My organization plans to upgrade our current implementation from the current release 12.0.4 to 12.1.3. Can someone please share some information in this regard. I have already found a note on EBS Data Model Comparison and am looking for some additional information which can help us with a detailed process.
