Requsition stuck in Pre-approved status.

Could anyone help me with the following issue please.

I have a Requsition created in iprocurement.  It was stuck in a status of pre-approved. with the error 3205: 'ABORNACELLI' is not a valid role or user name.

This user had been end dated,  so I push the end date forward and retried the workflow.  The requsition is now approved.

What I need to understand is why the user ABORNACELLI was included in the approval process, as I cannot see her supervising anyone in HR.

Can anyone suggest where I start looking.
