Help with Ops Center - Task not registered on <server>

I am very new to using Ops Center. I have the documentation and have created an operational plan and profile. I ran it on a server, but always receive the error "Task not Registered on <server>" and it dies after seconds. What am I missing? The documentation for Ops Center is very hard to find things in. I haven't found a section on errors for tasks. I'd appreciate being pointed to good documentation or advice about this issue.Thanks.
